REGISTRATION FEE: $1,050
Student rate: $750 (A proof of current student status is required to complete registration)
Discounted rate available for members of the University of Michigan Institute for Healthcare Policy and Innovation (IHPI), University of Michigan Mixed Methods Program (UM-MMP) or U-M Department of Family Medicine. Upon completion of the registration form, please email a proof to Satoko Motohara at smotohar@umich.edu (proof of student status or membership is required to complete registration).

Withdrawal and Refund Policy:
Participants may withdraw from the workshop and receive a full refund if done before October 12, 2021. Withdrawals done between October 13 and November 1, 2021 will be subject to a late withdrawal fee of $250 for the Mixed Methods Workshop and $50 for the Qualitative Workshop. There will be no refunds for withdrawing on or after November 2, 2021.
