Non-Student rate: $1,200
Group rate: $810 per person (when you register with 5 or more before May 22, 2019)

Discounted rate available for members of the University of Michigan Institute for Healthcare Policy and Innovation (IHPI), University of Michigan Mixed Methods Program (UM-MMP) or U-M Department of Family Medicine. Please email Satoko Motohara at [email protected] for discounted rates (proof of student status or membership is required).
The registration fees include your attendance to the 2.5 day interactive workshop, direct feedback from the workshop instructors on your project, a USB drive with workshop documents, breakfast and lunch.

- If you are an international participant and would prefer to pay via international wire transfer, please complete the Registration Form and submit Payment by International Wire Transfer Form.
Withdrawal and Refund Policy:
Participants may withdraw from the workshop and receive a full refund if done before May 22, 2019. Withdrawals done between May 23 and June 11, 2019 will be subject to a late withdrawal fee of $250. There will be no refunds for withdrawing on or after June 12, 2019.
